Family Caregiver Support Program

Provides information about resources that will help families in their caregiver roles.
Assists caregivers in locating services from a variety of private pay and voluntary agencies.
Offers ongoing caregiver education and training opportunities.
Facilitates counseling and peer support to help caregivers cope with the stress of care giving and the disabling effects of a family member’s chronic condition.
Provides limited supplemental support services to fill a service gap that cannot be filled in any other manner.
Eligible Individuals
Eligibility Criteria
Adult family member, or another individual, who is an informal provider of in-home and community care to an adult over 60 or to a person with Alzheimer's disease
An individual over 55 providing care to a person with a developmental disability.
Care receiver must be unable to perform 2 or more activities of daily living.
Caregiver must live in the home or provide a substantial amount of in-home care.
